ELIGIBILITY

Services for Children

Eligibility for services from the Huron County Board of Developmental Disabilities is determined a little differently for each of several age groups, and is the responsibility of the Intake & Eligibility Specialist for those aged three and up.

Birth to age 3:

For Home Visiting, we are Prenatal to age 3. Our eligibility is as follows:

  • Prenatal woman or parent of infants and toddlers under the age of 24 months at the time of enrollment
  • Eligible for WIC or Medicaid
  • OR under the 200% Poverty Guideline
For Early Intervention, we are birth through third birthday. Our eligibility is as follows:
  • Children who have a documented diagnosed physical or mental condition with a high probability of resulting in a developmental delay
  • Children who have a delay demonstrated through a multi-disciplinary evaluation
  • Children who demonstrate a "need for services" through an assessment

Ages 3 through 5:

Requires current confirmation from a qualified clinician of a diagnosed condition that is severe and chronic, not solely caused by mental illness, likely to continue indefinitely, and results in at least TWO developmental delays.

The disability causes the individual to need a combination and sequence of special, interdisciplinary, or other type of care, treatment or provision of services for an extended period of time that is individually planned and coordinated for him or her.

Ages 6 through 15:

Requires current confirmation from a qualified clinician of the diagnosis of a severe, chronic disability, not solely caused by mental illness, that is likely to continue indefinitely.

The disability causes the individual to need a combination and sequence of special, interdisciplinary, or other type of care, treatment or provision of services for an extended period of time that is individually planned and coordinated for him or her.

After this diagnosis verification is received by HCBDD, along with all other required documentation, a Children's Ohio Eligibility Determination Instrument (C/OEDI) assessment will be scheduled and completed.

The results of the C/OEDI must show that the individual has a substantial functional limitation (SFL) in at least THREE or more of the following six life areas:

  • Self-care
  • Expressive/receptive language
  • Learning/cognition
  • Mobility
  • Self-direction
  • Capacity for independent living

Services for Transition Youth & Adults

Eligibility for services from the Huron County Board of Developmental Disabilities is determined a little differently for each of several age groups, and is the responsibility of the Intake & Eligibility Specialist for those aged three and up.

The Ohio Department of Developmental Disabilities provides an assessment tool for individuals from the age of 16 through adulthood; the Ohio Eligibility Determination Instrument (OEDI).

Ages 16 and up:

Current confirmation from a qualified clinician of the diagnosis of a severe, chronic disability, not solely caused by mental illness, manifested prior to the age of 22, and likely to continue indefinitely.

The disability causes the individual to need a combination and sequence of special, interdisciplinary, or other type of care, treatment or provision of services for an extended period of time that is individually planned and coordinated for him or her.

After this diagnosis verification is received by HCBDD, along with all other required documentation, an OEDI assessment will be scheduled and completed.

The results of the OEDI must show that the individual has a substantial functional limitation (SFL) in at least THREE or more of the following seven life areas:

  • Self-care
  • Expressive/receptive language
  • Learning/cognition
  • Mobility
  • Self-direction
  • Capacity for independent living
  • Economic self-sufficiency
