Transition Self-Advocacy

HCBDD Transition SSA’s provide individuals with developmental disabilities the training and resources needed to ensure a smooth, successful transition from school to work.

Rather than wait until a student reaches their senior year in high school before looking at possible careers, Transition SSA’s begin working with individuals as young as age 14 and up to age 22.

Transition SSA’s will coordinate and monitor services for Transition Youth. This includes, but is not limited to, working with OOD, OOD vendors, all area schools, HCBDD SSA’s, individuals and families.

The goal is to give individuals an opportunity to explore the many careers available to them before making a decision. This results in a more secure decision by the individual and a happier, more productive employee.

Project Life

Project LIFE is more than a typical transition-to-work program. This comprehensive program combines classroom and life skill education with authentic work experiences to prepare students with disabilities for a self-determined future.

Tiers of Personalized Preparation

Two levels of transition-focused course of study assessments in combination with a comprehensive employment skill scoring rubric and work performance report, show the data that will provide information for a smoother transition to adulthood and employment.
